Job summary

Upperline Health, Inc. is seeking a Nurse Practitioner to provide clinic-based care in the Metro Atlanta market, collaborating with a local physician team.

APPs develop complex care plans, provide diagnostics and treatments in clinic, home, or via telehealth, and educate patients and families on medications and prevention. Qualifications

• Master’s degree in nursing required.
• Board Certification in NP specialty (Adult/Adult-Geriatric/Family) required.
• DEA registration or eligibility required.
• 6 months NP experience in Primary Care/Internal Medicine required.
• Value-based care experience preferred.
• Must be comfortable providing care in patients’ homes.
• Experience with AthenaHealth EMR preferred.

Responsibilities

• Develop and implement clinical plans of care for adult patients with chronic conditions.
• Gather patient history, perform exams, and diagnostics in clinic, telehealth, or home visits.
• Develop and implement treatment plans aligned with patient goals and conditions.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Provide compassionate care for complex patients and families.
• Educate patients and families about medications, diet, and prevention.
• Coordinate with other Upperline clinicians and community providers for optimal care.

6 months NP experience in Primary Care

Value-based care experience

AthenaHealth EMR experience

Excellent oral and written patient/col

Education
