Job summary
Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our Metro Atlanta market at the Austell Clinic. In collaboration with the local physician team, the APP develops complex care plans, delivers diagnostics and treatments, and educates patients and families on medications, diet, and prevention in clinic, home, or via telehealth.
Ideal candidates have primary care/internal medicine experience or value-based care exposure.
Qualifications
- Master’s degree in nursing is required.
- Board certification as NP (Adult/Adult-Geriatric/Family) is required.
- Current State RN & NP license required.
- DEA registration or eligibility required.
Responsibilities
- Develop and implement clinical plans of care for adults with chronic conditions.
- Gather history, exams, and diagnostics in clinic, telehealth, or home visits.
- Collaborate with interdisciplinary team for patient and family needs.
- Prescribe medications as allowed by state nursing board.
- Maintain accurate medical records and documentation.
