Job summary

Oak Street Health in Jamaica, NY is seeking a Nurse Practitioner to provide value-based primary care for adults on Medicare. You will work on a collaborative care team, treating patients independently with physicians and other clinicians, and focusing on long-term health relationships and preventive care.

The role emphasizes care coordination, documentation, and population health within outpatient settings, reporting to the Center Medical Director.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the care team.
• Assist with phone triage and outreach.
• Educate patients on health conditions, care plans and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
