The position focuses on preventive care, care transitions, and coordinating with the care team to ensure timely follow-ups, documentation, and adherence to protocols. Strong communication and organizational skills are essential.

Formación

• High school diploma required.
• Minimum of 3 years of clinical or home health experience.
• Knowledge of CMS, ACOs, HEDIS; payor cost sharing preferred.
• Knowledge of physician office practice operations; 1 year preferred.
• Proficiency in keyboarding and Epic EHR.

Responsabilidades

• Meets competency expectations for leadership at the appropriate level.
• Identify quality gaps and participate in Quality Improvement Programs.
• Assist patients with scheduling preventive screenings and appointments; maintain communication with providers.
• Conduct telephonic outreach to close care gaps and address medication adherence.
• Counsel patients and educate families using standardized protocols; act as subject matter expert in care transitions.
• Document information using standard assessment tools and maintain chart compliance.
