CHRISTUS Health in Irving, TX is seeking a Licensed Vocational Nurse (LVN) to provide care coordination under the Population Health Director of Care Management. You will follow physician-approved patient protocols to optimize health and reduce readmissions, working closely with the outpatient care team.

In this role, you’ll collaborate with Nurse Navigators and the care team to manage high-risk patients with complex needs, ensuring timely preventative care and accurate documentation.

• Manages a case load with identified gaps in care in collaboration with the Health Care Team.
• Coordinates preventative/follow-up care for patients from participating providers.
• Reviews patient medical records to identify care gaps and documents encounters.
• Schedules needed preventative services using physician-approved protocols.
• Assists in transition of care outreach to discharged patients to schedule return visits with the PCP.
• Communicates the importance of recommended procedures and testing to patients and families.
• Maintains confidentiality and all required CEUs in compliance with state and regulatory requirements.
