Responsibilities

• Identify special needs members through health screens and other resources.
• Review and evaluate requests for specialized and private care.
• Act as a clinical resource to complex medical cases.
• Act as a liaison between members, providers, and pharmacies.
• Coordinate with Discharge Planning teams for member placements.
• Ensure follow-up on care and service coordination.
• Assess service needs and determine support available.
• Oversee the development and modification of service plans.
• Serve as clinical consultant for the Service Coordination team.
