• Strong understanding of value-based care principles and practices.
• Excellent analytical skills with the ability to interpret complex data and make strategic decisions.
• Experience with Salesforce or similar CRM platforms is preferred.

Responsibilities

• Evaluate provider practices to determine capabilities in value-based care and identify growth areas.
• Engage in assessment, root cause analysis, strategy development, implementation, and outcomes tracking.
• Assist providers with action plans focusing on population health management, satisfaction, and cost performance.
• Lead collaborative meetings with provider staff and internal stakeholders for alignment.
• Create and deliver tailored learning opportunities for implementing interventions.
• Educate providers on proprietary tools, data, and technology to improve performance metrics.
• Utilize data analytics to monitor key performance metrics for assigned providers.
• Document action plans and provider activities in Salesforce for reporting.
• Cultivate long-term relationships with physicians and healthcare systems to support improvements.
• Advise executives on segment-specific strategies in financial performance and quality management.
• Exercise independent judgment on complex issues with minimal supervision.
