Qualifications

• Experience as a Primary Care Physician.
• Commitment to providing quality patient care for adult population.

Responsibilities

• Outpatient medicine, continuity of care, health maintenance, and disease prevention.
• Independently performing comprehensive health assessments and communicating care plans to patients and their families.
• Diagnosing, treating, and managing Medicare Advantage patients with multiple chronic conditions.
• Collaborating with the care team to develop plans of care and minimize unnecessary tests or fragmented care.
• Complying with state scope of practice regulations.
