Job summary

Compassus is seeking a Care Liaison Manager to model our values and advance value-based care coordination across Hospice and Home Health services within a defined geographic area. You will lead strategic engagement with hospital systems, align JV and local leaders, and drive cross-team collaboration to reach shared VBE goals.

The role requires healthcare leadership experience, strong relationship-building, and the ability to manage multiple projects in a dynamic environment.

Qualifications

• Bachelor's Nursing or Master's Social Work required.
• Bachelor's in healthcare, business admin or communications or equivalent.
• Master's degree in clinical, business, health administration or related area preferred.
• 3+ years complex healthcare business project management experience.
• 3+ years managing healthcare teams, business development or program management preferred.
• Previous business development in state healthcare marketplace preferred.
• Experience collaborating with healthcare executives and decision makers preferred.

Responsibilities

• Lead and coordinate care coordination activities across Hospice and Home Health services.
• Engage hospital systems and JV leaders to align on value-based goals.
• Provide leadership and connectivity for JV's home health and hospice service lines.
• Develop and implement market strategy to drive growth and collaboration.
