Job summary

Medical City Heart & Spine Hospitals in Dallas, TX, is seeking a Registered Nurse for a full-time day shift to coordinate care for heart failure and VAD/transplant patients. You will collaborate with physicians, the transplant team and community providers to tailor care plans and ensure seamless patient flow.

Qualifications include a TX RN license, BSN within two years, BLS and ACLS. At least two years in critical care and transplant/VAD experience are preferred; on-call duties are required.

Qualifications

• Graduate of accredited school of professional nursing.
• BSN must be obtained within 24 months of hire.
• Current RN license in the State of TX or a compact license.
• Current BLS certification from AHA or Red Cross.
• ACLS certification required within 6 months of hire.
• At least two years of critical care experience.
• Experience in management of organ failure/transplant/VAD patients preferred.
• On-call responsibilities to support the program.

Responsibilities

• Liaise with heart failure patients, the pre-transplant Heart Transplant Coordinator, the VAD Program Supervisor and community care providers to match patient needs with program offerings.
• Coordinate information flow among medical, nursing, financial and insurance staff.
• Represent heart failure, VAD and transplant programs in hospital and community activities.
• Maintain transplant/VAD datasets and registries (Intermacs and related registries).
• Support VAD Process Improvement Committee preparations as required.
• Assist with Joint Commission requirements for VAD certification.
• Educate patients, families, staff and the community as needed.
• Manage equipment, supplies and charges with hospital.
