Job summary

Mercy General Hospital in Sacramento seeks a Circulatory Device Coordinator RN to lead the care and coordination of ventricular assist device patients across the care continuum, collaborating with the Advanced Heart Failure and Circulatory Device Program Medical Director.

You will design VAD clinic services, drive performance-improvement initiatives, and serve as a clinical liaison to physicians, staff, and patients/families, with responsibility for documentation and policy adherence while

Qualifications

BSN or higher and CA RN license.
5 years of professional clinical experience in critical care.
1 year of experience caring for Ventricular Assist Device (VAD) patients.
BLS certification (AHA).
ACLS certification (AHA).
CA RN licensed with current credentials.

Responsibilities

Oversee care for high-risk VAD patients across acute to home settings with Medical Director and multidisciplinary team.
Design and implement VAD Clinic services and performance improvement programs.
Provide 24/7 availability to support the clinical management team.
Coordinate referrals, testing, consults, and hospital procedures for VAD patients.
Develop therapeutic relationships with patients and families and facilitate access to community resources.
Educate acute care staff on VAD care and related protocols.
