Job summary

Aptive Resources is hiring twelve Registered Nurses to support the Patient Business Office / Community Care program at the Dayton VA Medical Center. This is a clinic-based RN role focused on care coordination rather than bedside care, reviewing consults, coordinating follow-ups, and keeping records complete for timely veteran care.

Responsibilities include managing labs, imaging, and referrals, communicating with VA providers and community clinicians, and documenting all care in CPRS/EHR.

Qualifications

• Graduate of an ACEN/CCNE-accredited nursing program and state-approved nursing license.
• Current, unrestricted RN license in Ohio.
• BLS and CPR certification; ACLS required for specialty clinics.
• Fluent in English, able to pass VA credentialing and background check.

Responsibilities

• Provide nursing care coordination for an assigned panel of Veterans; assess needs and plan nursing care.
• Own follow-up on labs, imaging, and procedures; conduct RN phone consultations.
• Communicate with referring clinicians and community providers about consults.
• Obtain patient history, plan and reassess nursing care as needs change.
• Instruct patients on medications and therapies; document teaching.
• Verify orders and review lab data in the VA EHR; document care.
• Administer medications and IV therapies within scope; act during emergencies.
• Document all care in CPRS/EHR before shift end; select E/M and procedure codes.
• Report safety events and patient complaints within required timelines.
• Participate in discharge planning, team huddles, and required meetings.
