Job summary

Qualifications

Responsibilities

• Plan, develop, assess, and evaluate care provided to members.
• Collaborate with physicians and the multidisciplinary health care team to develop individualized plans of care.
• Recommend alternative levels of care and ensure compliance with regulations.
• Assess high-risk patients in need of post-hospital care planning.
• Develop and coordinate discharge plans to meet patient needs.
• Review and coordinate patients' hospital stays to ensure timely service delivery.
• Participate in Bed Huddles and implement recommendations based on patient needs.
• Coordinate continuity of care, including utilization management and discharge planning.
• Conduct daily clinical reviews for utilization and quality management.
• Act as a liaison between inpatient facilities and referral agencies.
