Job summary

Healthier Mississippi People LLC seeks a qualified RN to assess, develop, implement, and evaluate nursing care for patients in an assigned clinical area. You will coordinate patient scheduling, records, admissions, and liaise with case managers and home health teams to ensure seamless care delivery.

Qualifications

• Valid RN license is required.
• Experience in a clinical nursing setting is preferred.
• Ability to coordinate care across multiple care settings and teams.

Responsibilities

• Provides patient care by performing assessments, planning, implementing and evaluating treatments and interventions.
• Documents care into the patient’s electronic medical record.
• Coordinates delivery of care within the practice and across health care settings.
• Administers and coordinates resources and direct clinical activities and workflow.
• Fosters interdisciplinary collaboration to improve quality of care and patient satisfaction.
• Communicates with patients, caregivers, and other health care professionals about patient care and the nurse's role.
• Ensures access to appropriate, safe, effective, timely and equitable care.
• Mentors new staff and participates in orientation and ongoing education.
• Maintains patient confidentiality and reports concerns to supervisor.
• Demonstrates exceptional customer service standards.
