Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within the scope of practice, ensuring patient needs are met per standards and physician orders. The role focuses on charting, incident reporting, and coordination with physicians during rounds.

Ideal candidates have Long Term Care experience, CA LVN/LPN licensure, and BLS/CPR. Experience in nursing management is preferred but not required, with compliance to all policies and guidelines.

Qualifications

• Long Term Care experience required.
• Current LVN/LPN license in practicing state.
• BLS/CPR certification required or willingness to acquire.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
