Job summary

Sweetwater Care in Fresno, CA seeks an LVN/LPN to provide nursing care within scope, ensuring patient needs align with standards and physician orders. Responsibilities include supervising nursing assistants, charting, and coordinating admissions/discharges.

The role requires current LVN/LPN licensure in California, BLS/CPR certification, and experience in long-term care. Knowledge of charting procedures and regulatory compliance is essential.

Qualifications

• Has experience in long term care settings.
• Proficient in Point Click Care charting.
• Active LVN/LPN license in the practicing state; BLS/CPR certification preferred or willing to obtain.

Responsibilities

• Lead the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established
• Admit, transfer, and discharge residents as necessary
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.
