PASCO, the Personal Assistance Services of Colorado, seeks an Unskilled Services Coordinator to support IHSS and Personal Care Services programs. You will advocate for clients, coordinate care plans, and assist families navigating Medicaid and waiver services.

The role requires experience in case management, strong customer service, and the ability to travel to Lakewood for home visits. Bilingual Spanish is highly preferred, though not required.

Qualifications

• Experience in Case Management or Client Care Coordination; familiarity with Medicaid or IHSS is a big plus.
• 2+ years in client-facing roles delivering high levels of service.
• Tech-savviness with Microsoft Office, Outlook, and EHR systems.
• Able to work independently in a fast-paced healthcare environment.
• Willingness to travel for home visits in the Lakewood area.

Responsibilities

• Advocate and coordinate with case management entities to develop personalized care plans for PASCO clients.
• Conduct on-site supervisory visits and provide feedback on PCWs and Homemakers.
• Assist families in navigating Medicaid and waiver services.
• Collaborate across PASCO departments to streamline processes.
• Maintain compliant documentation in EHR and meet state policies.
• Build strong community partnerships and support clients, families, and caregivers.

Schedule: Hybrid (Tuesdays & Thursdays - Lakewood office, Mon/Wed/Fri - field visits)

What You'll Do

• Advocate & Coordinate: Work with case management entities to develop personalized care plans for PASCO clients, ensuring all needs are met.
• Ensure Quality Care: Conduct on-site supervisory visits and provide feedback on Personal Care Workers (PCWs) and Homemakers.
• Support Clients & Caregivers: Act as a trusted resource to help families navigate Medicaid and waiver services.
• Collaborate Across Teams: Work with PASCO departments, including Medical Records, Prior Authorization, HR, and Intake to streamline processes.
• Maintain Compliance & Documentation: Keep records updated in Electronic Health Record (EHR) systems and ensure compliance with PASCO and state policies.
• Build Relationships: Establish strong community partnerships and act as a go-to support for clients, families, and caregivers.

What You Bring To The Team

• Experience in Case Management or Client Care Coordination – Familiarity with Medicaid, Home Healthcare, or IHSS is a big plus!
• Customer Service & Relationship Management Skills – At least 2 years of experience in client-facing roles, ensuring high levels of satisfaction.
• Tech-Savviness – Proficiency in Microsoft Office Suite, Outlook, and Electronic Health Record (EHR) systems.
• Self-Motivation & Adaptability – Ability to work independently and in a fast-paced, evolving healthcare environment.
• Willingness to Travel – Conduct home visits in the Lakewood area to ensure high-quality care.
• Bilingual in Spanish (huge plus but not required)
