PASCO (Personal Assistance Services Of Colorado) is hiring an Unskilled Services Coordinator to champion IHSS and Personal Care Services in North Eastern Colorado. The role blends field visits with monthly office time in Lakewood, and pays $22–$25/hour based on experience.

You'll coordinate care plans, support families navigating Medicaid, and collaborate across departments to ensure quality, compliant service. Bilingual in English and Spanish is highly preferred.

Qualifications

• Experience in Case Management or Client Care Coordination with Medicaid, IHSS, or home healthcare.
• Strong customer service and relationship-building skills in client-facing roles.
• Proficiency with Microsoft Office Suite, Outlook, and Electronic Health Record (EHR) systems.

Responsibilities

• Advocate & Coordinate: Develop personalized care plans with case management entities to meet PASCO clients’ needs.
• Ensure Quality Care: Conduct on-site supervisory visits and provide feedback on Personal Care Workers and Homemakers.
• Support Clients & Caregivers: Help families navigate Medicaid and waiver services.
• Collaborate Across Teams: Work with PASCO departments to streamline processes and maintain compliance.
• Maintain Compliance & Documentation: Keep records updated in EHR systems and ensure policy compliance.
• Build Relationships: Establish strong community partnerships and be a trusted support for clients and families.

What You’ll Do

• Advocate & Coordinate: Work with case management entities to develop personalized care plans for PASCO clients, ensuring all needs are met.
• Ensure Quality Care: Conduct on-site supervisory visits and provide feedback on Personal Care Workers (PCWs) and Homemakers.
• Support Clients & Caregivers: Act as a trusted resource to help families navigate Medicaid and waiver services.
• Collaborate Across Teams: Work with PASCO departments, including Medical Records, Prior Authorization, HR, and Intake to streamline processes.
• Maintain Compliance & Documentation: Keep records updated in Electronic Health Record (EHR) systems and ensure compliance with PASCO and state policies.
• Build Relationships: Establish strong community partnerships and act as a go-to support for clients, families, and caregivers.

What You Bring To The Team

• Experience in Case Management or Client Care Coordination – Familiarity with Medicaid, Home Healthcare, or IHSS is a big plus!
• Customer Service & Relationship Management Skills – At least 2 years of experience in client-facing roles, ensuring high levels of satisfaction.
• Tech-Savviness – Proficiency in Microsoft Office Suite, Outlook, and Electronic Health Record (EHR) systems.
• Self-Motivation & Adaptability – Ability to work independently and in a fast-paced, evolving healthcare environment.
• Willingness to Travel – Conduct home visits in the Lakewood area to ensure high-quality care.
• Bilingual in Spanish (huge plus but not required)
