Avantara Mountain View is seeking an experienced LPN to serve as Unit Manager Nurse. You will lead admissions, collaborate with hospital liaison, and ensure smooth referral-to-admission flow with attention to resident safety and quality care.

Qualifications

• Unencumbered South Dakota LPN license is required.
• Minimum 1 year of clinical experience.
• Excellent written and verbal communication skills.
• Preferred experience with Point Click Care (PCC).
• Team oriented with analytical and multitasking abilities.
• Knowledge of Medicare/Medicaid/Managed Care Organizations a plus.
• Experience performing audits or quality assurance is advantageous.

Responsibilities

• Lead all admissions and oversee the resident admission process.
• Collaborate with Hospital Liaison and admissions teams on referrals.
• Review referrals for clinical appropriateness and order entry.
• Prepare resident rooms and ensure necessary records for admission.
• Ensure high standard of care and resident safety.
