Job summary

Avantara Mountain View is seeking an experienced Unit Manager Nurse to lead admissions and coordinate the resident intake process. You will review referrals, enter orders, and ensure all necessary records are obtained for a seamless admission experience while coordinating with families to finalize documentation.

The role emphasizes attention to detail, clinical oversight, and collaboration with the admissions team to prepare resident rooms and ensure a smooth transition for new residents.

Qualifications

• RN or LPN license required (SD).
• At least 1 year of clinical experience.
• Excellent written and verbal communication skills.
• Preferred experience with Point Click Care.
• Team oriented, analytical, and able to multi-task.
• Basic knowledge of Medicare/Medicaid/Managed Care Organizations.
• Experience performing audits or quality assurance is a plus.

Responsibilities

• Lead the resident admissions process from referral to room readiness.
• Collaborate with Hospital Liaison and admissions team to review referrals.
• Ensure admissions documentation is complete and accurate.
• Oversee admission checks to maintain high standard of care for residents.
• Support smooth, organized onboarding for new residents and their families.

As a Unit Manager Nurse, you will take the lead on all admissions and ensure the entire resident admission process is smooth and effortless. You will work with the Hospital Liaison and admissions team to clinically review referrals who will potentially admit to our facility. You will play a key role to ensure our residents receive the high standard of care they have grown to expect at Avantara Mountain View by overseeing the resident admissions process from the referral process, entering orders, ensuring all records necessary for admission are obtained, working with residents and their family members to complete admission documentation, and ensuring new resident’s rooms are appropriately prepared for them prior to their arrival. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
To be eligible for consideration applicants should have:

• As a minimum, an unencumbered State of South Dakota Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license.
• At least 1 year of prior clinical experience.
• Excellent written and verbal communication skills.
• Preferred experience with Point Click Care.
• Team oriented, analytical, and ability to multi-task.
• Basic knowledge of Medicare/Medicaid/Managed Care Organizations.
