Job summary

Centers Plan for Healthy Living is seeking a Care Coordinator to work with a multidisciplinary team to support members in the Managed Long Term Care program and related lines of business. The role focuses on scheduling, communication, and documentation to ensure timely and coordinated care.

Responsibilities include managing UAS assessments, meeting daily quotas, preparing reports, and serving as an advocate between members, families, physicians, and the care team.

Qualifications

• 2 years customer services experience
• Strong written and verbal communication skills
• Computer proficiency
• Strong multi-tasking abilities
• Experience in managed care (preferred)
• Bilingual (preferred)

Responsibilities

• Assist in scheduling of ongoing member visits and track/cancel as needed
• Meet daily scheduling quotas
• Provide a daily report of all scheduled visits
• Upload UAS documents into member files
• Generate member correspondence and mailings
• Coordinate communication between Field Assessors and the care team
• Document daily calls and care coordination interventions/outcomes
• Liaise between member/family, physicians and the care team for scheduling assessments
• Participate in ongoing education activities
• Ensure compliance with policies and regulations
• Other assigned duties
