Job summary

Personal Touch Home Aides of New York is seeking a dedicated UAS Assessment Nurse (RN) to conduct in-person assessments for clients in Brooklyn, NY. The role requires a valid NY RN license and UAS experience, with per-visit compensation as the pay structure.

Qualifications

• RN licensed and registered to practice in New York State
• UAS-NY Certification required
• Experience in Home Care or case management preferred

Responsibilities

• Conduct UAS assessments to determine level of care and support needs
• Utilize the UAS tool to document results accurately
• Provide compassionate professional interactions in community settings
• Submit assessments within designated timeframes and maintain records
• Collaborate with other care providers to ensure comprehensive assessment
• Attend required training and stay updated on assessment changes
