Personal Touch Home Care in Brooklyn, NY seeks an experienced UAS Assessment Nurse (RN) to perform in-home assessments and determine client eligibility and ongoing needs. A valid NY RN license and UAS experience are required, with rates paid per visit.

The role involves conducting thorough UAS assessments, documenting results, and coordinating with care teams to support patient care plans. Per diem, fee-for-service positions with competitive per-visit compensation are offered.

Qualifications

• Registered Nurse, currently licensed and registered to practice in New York State.
• UAS-NY Certified.
• Bachelor’s degree in nursing preferred.
• Experience in Home Care, case management, or long-term home care.
• Demonstrated skills in planning, organizing and directing the work of others required.
• Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.
• Proficient with DOH UAS assessment instrument.

Responsibilities

• Conduct UAS assessments for clients to determine their level of care and support needs.
• Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately.
• Provide compassionate and professional interactions with clients in various community settings.

This is a fee-for-service position. In order to be considered for this job you MUST have a valid RN license in the state of NY and prior UAS experience.

Rates are paid per visit.

Pay: Initial visit- $165.00 per visit; Reassessment- $140.00 per visit

JOB SUMMARY
