Four Seasons Home Care is a growing home care agency serving the five boroughs of NYC. This role focuses on outreach, referrals, and community partnerships to expand patient enrollment and program awareness.

The position requires up to 75% travel within New York City, fluency in English, Spanish, and Creole, and familiarity with NY Medicaid and long-term care programs. A car is a plus and prior LHCSA/CHHA experience is preferred.

Qualifications

• High school diploma or college degree in related business field preferred.
• Up to 75% travel within New York City; having a car is a plus.
• Trilingual in English, Spanish, and Creole required.
• Knowledge of NY Medicaid and managed Long Term Care is a plus.
• 1–2 years of LHCSA/CHHA/SNF/ILF experience preferred.

Responsabilités

• Obtain referrals from existing referral sources; follow up on referrals to enroll patients.
• Build partnerships and techniques to attract new clients in the industry.
• Schedule and present information about Four Seasons programs to community organizations and healthcare professionals
• Utilize marketing strategies to attract new patients
• Research and participate in health and wellness events in NYC; coordinate partnerships with health agencies
• Attend community events as needed
• Develop outreach materials in collaboration with Branding department
• Demonstrate knowledge of agency, vision, mission, and values; differentiate from competitors
• Provide monthly reports on outreach efforts
• Perform other related marketing duties as assigned
