Job summary

Ohio's Hospice is seeking a compassionate Registered Nurse (RN) to join our team as a Full-Time Triage Nurse. The role focuses on clinical support, patient/family calls, and coordination of care in our Call Center, delivering timely, compassionate service.

The RN will triage calls, prioritize needs, and communicate with the interdisciplinary team to ensure continuity of care, comfort, dignity, and quality of life for hospice patients.

Qualifications

• Must hold a current RN license in Ohio.
• CPR certification is required.
• Valid driver’s license with a safe driving record.
• State-required auto insurance coverage.
• Strong oral and written documentation and computer skills.
• Excellent clinical assessment and judgment abilities.
• Experience with pain and symptom management preferred.
• IV and vascular access device skills.
• Ability to perform basic mathematical calculations (whole numbers, fractions, decimals, ratios, rates, percentages).

Responsibilities

• Receive and triage patient and family calls based on clinical urgency and individual needs.
• Provide professional, compassionate customer service and clinical guidance.
• Assess patient needs and coordinate services per the care plan.
• Communicate patient condition changes to clinical staff.
• Coordinate changes in level of care with schedulers and departments.
• Document assessments, interventions, observations, and communications in the EMR.
• Maintain accurate call logs and inform the interdisciplinary team.
• Support continuity of care to promote comfort, dignity, and quality of life.
