Job summary

Patriot Homecare is hiring a Hospice Triage Registered Nurse to join our compassionate team in Ohio. You will answer calls, perform triage, assess patient needs, and guide care decisions within the hospice plan.

The role emphasizes education, coordination with clinicians, and supporting families and caregivers while maintaining high standards of care. A BSN and Ohio RN license are required, with hospice experience preferred.

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Must be licensed in the State as a Registered Nurse.
• One or more years of experience in hospice preferred.
• Must have knowledge of Medicare and Medicaid guidelines.
• Must have a criminal background check.
• Must have a CPR (BLS) certification.

Responsibilities

• Answer incoming calls from families, caregivers, facilities, physicians, and staff.
• Perform telephone triage and assess patient condition, symptoms, pain, and care needs.
• Determine appropriate level and urgency of care based on hospice plan.
• Provide education, reassurance, and clinical guidance within hospice scope.
• Coordinate referrals and admissions with clinicians and leadership.
• Document triage calls and follow-ups accurately.
• Direct field staff to patient visits based on acuity and location.
• Maintain timely, accurate clinical documentation.
