Capital City Hospice in Columbus, OH is seeking an experienced Registered Nurse (RN) for Hospice On Call / Triage. You will triage after-hours patient and family calls, assess needs, update care plans, and coordinate with the interdisciplinary team.

The role requires an Ohio RN license, CPR and the ability to handle emotional stress. You’ll work with a compassionate local team in a community-focused setting that values quality care and staff support.

Qualifications

• Graduate from an accredited registered nursing program.
• Possession of a current license to practice in OH as a Registered Nurse.
• Minimum of two years of varied work experience as a professional nurse.
• Demonstrated skill in nursing practices and clinical judgments.
• Must possess CPR Certification.
• Valid driver’s license and proof of insurance.

Responsibilities

• Triage after-hours patient and family phone calls, determining need for services including assessments and care plan updates.
• Obtain data on physical, psychological, social, and spiritual factors impacting patient and family needs and coordinate interventions with the team.
• Initiate communication with attending physician and interdisciplinary staff to coordinate optimal care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate clinical records with care plans and progress notes.
