Job summary

Charlotte Community Health Clinic is seeking a Triage Nurse to assess patients, determine urgency, and direct them to the appropriate care area. The role supports wound care and nursing tasks under the Director of Nursing in a full-time capacity.

Applicants should be a Graduate of an accredited RN program with NC licensure (compact license allowed), CPR certification, and 1–2 years in clinical settings preferred. Epic EMR experience preferred; bilingual skills are a plus.

• Able to communicate effectively with a wide range of staff, physicians and public.

• Maintains confidentiality and follows HIPAA guidelines.
• Excellent organizational, problem solving and critical thinking skills.

Responsibilities

• Assess patients and determine urgency to direct them appropriately.
• Document patient needs, actions taken and responses in medical records.
• Provide age-specific nursing care and assist with wound care as needed.
• Administer medications and treatments within scope of practice.
• Coordinate referrals and communicate with providers to support care plans.
• Ensure patient safety and advocate for patient needs.
