Job summary

Charlotte Community Health Clinic is seeking a Triage Nurse to assess patients, determine urgency, and direct them to the appropriate care area. The role supports wound care and nursing tasks under the Director of Nursing in a full-time capacity.

Applicants should be a Graduate of an accredited RN program with NC licensure (compact license allowed), CPR certification, and 1–2 years in clinical settings preferred. Epic EMR experience preferred; bilingual skills are a plus.

Job Summary

Key Responsibilities
  • Provides age-specific patient care by assessing, implementing, evaluating, and documenting the individual patient needs, actions taken and the patient responses relative to nursing care;
  • Shares patient information with provider to facilitate care evaluations and updates data in the medical record; Accepts direction from provider; Makes patient referrals as directed by the provider, and as appropriate, to resolve identified problems or reach established patient goals; Reviews orders for clarity and appropriateness;
  • Gathers information, reviews past allergic reactions;
  • Administers medications and treatments within scope of practice, uses approved policies/procedures/protocols;
  • Uses problem solving skills to identify and work through issues;
  • Displays critical thinking skills to determine appropriate care;
  • Considers patient/family cultural differences and incorporates into patient care;
  • Identifies patient learning needs, incorporates into care by providing education and counsel to patients, and documents patient understanding;
  • Knowledgeable of growth and development of individual’s life stages;
  • Helps to facilitate care provided is consistent with policies, procedures and telephone triage protocols, in cooperation with the physician;
  • Able to function in emergency situations and facilitates the care provided for all patients;
  • Facilitates quality of patient care;
  • Serves as an advocate for the patient;
  • Maintains all appropriate licensure.
  • Understands and follows HIPAA guidelines and ensures the confidentiality of patient results;
  • Ensures all test results, in-house and referred, are reported to the provider by checking a manifest or providing oversight of pending labs;
  • Able to perform CLIA waived tests;
  • Other duties as assigned
Minimum Qualifications

Able to communicate effectively. Interacts in a friendly, professional manner with a wide range of staff, physicians and public. Plan, prioritize and complete delegated task with attention to detail. Ability to multitask and work well under pressure is essential. Requires excellent organizational, problem solving and critical thinking skills. Requires the ability to maintain confidentiality. Must be able to function as part of a team. Comprehensive knowledge of Accounting procedures and principles. Ethical behavior when dealing with sensitive financial information. High level of accuracy and efficiency. Courteous, professional manner, strong customer service skills.

Experience

1-2 years in a physician practice or community-based health assignment preferred.

Additional Skills Required

Ability to work effectively with consumers from diverse social, cultural and economic backgrounds; Ability to work independently and use critical thinking skills; Excellent interpersonal & customer service skills; Strong oral & written communication skills; Demonstrates a positive attitude and respectful, professional customer service. Acknowledges patient’s rights on confidentiality issues, maintains patient confidentiality at all times, and follows HIPAA guidelines and regulations. Proactively acts as patient advocate, responding with empathy and respect to resolve patient and family concerns, and recognizes opportunities for improvement to meeting patient concerns. Proactively continues to educate self on providing quality care and improving professional skills. Ability to successfully complete generic and department specific skills validation and competency testing.

Additional Skills Preferred

Proficiency in electronic medical records; Experience with Epic (Preferred). Bilingual English/Spanish is preferred, but not required.

Education

Graduate of accredited Registered Nurse program required. Current RN Licensure in North Carolina May be a compact license that is legally allowed to practice in NC. CPR Certification.

Physical Requirements
  • Repetitive movement of hands and fingers – typing and/or writing.
  • Lifting of greater than 50 pounds
  • Occasional standing, walking, stooping, kneeling or crouching.
