Schedule : Friday 4:00 PM - Monday 8:00 AM

Location : REMOTE

Requirements of the Triage Nurse :

• Licensed Registered Nurse in the state of practice
• BLS Certified
• Excellence in communication and interpersonal skills to build rapport with patients and families
• Effective oral communication skills to interview patients and explain next steps, as well as to pass information to other members of the care team
• Familiarity with medical records, word processing, and email software programs

About the Triage Nurse role :

• Answer triage calls during assigned shifts while maintaining a calm, reassuring presence during emotionally intense situations
• Prioritize and respond to urgent calls promptly
• Provide guidance on managing hospice-related symptoms like pain, nausea, and difficulty breathing, recommending appropriate medication adjustments or interventions
• Collaborate with the interdisciplinary team including physicians, case managers, and other hospice staff to ensure seamless care coordination and follow-up
• Educate patients, families, and caregivers on the dying process, end-of-life care, and symptom management techniques
• Accurately document all patient interactions, interventions, and follow-up actions in the electronic medical record
• Update patient health information systems with notes
• Use clinical judgment to evaluate symptom severity and urgency, triaging patient and caregiver calls to determine appropriate care levels
• Provide emotional support and education to families and caregivers over the phone
• Assist caregivers with crisis management and problem-solving
• Ensure compliance with state, federal, and hospice agency regulations and protocols
• Perform any other tasks assigned
