Job summary

Phaxis is seeking an experienced Licensed Practical Nurse (LPN) for a fast-paced outpatient triage role in Huntingdon Valley, PA. This temp-to-perm opportunity offers the chance to transition into a permanent position based on performance.

The ideal candidate will manage telephone triage, patient concerns, and coordination with providers while maintaining accurate EMR documentation and ensuring HIPAA compliance. Strong clinical judgment and communication skills are essential.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Pennsylvania LPN license required.
• BLS certification required.
• ACLS certification preferred.
• 3+ years of related clinical experience preferred.
• Prior triage or telephone triage experience strongly preferred.

Responsibilities

• Conduct telephone triage and assess patient symptoms and concerns.
• Obtain accurate medical histories and relevant clinical information.
• Determine appropriate next steps using established clinical protocols.
• Communicate patient concerns and changes in condition to physicians.
• Respond to calls regarding test results, medication refills, and post-operative instructions.
• Provide patient education regarding medications, treatments, and follow-up care.
• Maintain accurate and timely documentation within the EMR.
• Ensure HIPAA compliance and organizational policies.
