Job summary

Community Health And Dental Care, Inc. (CHDC) in Pottstown, PA is seeking a Licensed Practical Nurse to provide direct patient care in an outpatient primary care setting. You will handle patient intake, vital signs, screenings, triage, and coordination with providers in a fast-paced environment.

The ideal candidate has 1+ year of clinical experience, EPIC EHR familiarity, and PA LPN licensure. Occasional travel to CHDC sites may be required; excellent patient service and teamwork are essential.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
• Current CPR certification or ability to obtain upon hire.

Responsibilities

• Prepare patients for provider visits by obtaining and documenting vital signs, medical history updates, medication reconciliation, and required screenings.
• Accurately document patient information and clinical findings in the electronic health record (EHR).
• Assist physicians, PAs, and NPs with examinations, procedures, and treatments.
• Administer medications, immunizations, injections, and other treatments as ordered and within licensure.
• Perform clinical procedures, including specimen collection, point-of-care testing, EKGs, wound care, and other nursing interventions.
• Assist in emergency situations and respond to urgent patient needs.
• Respond to patient calls, portal messages, and other communications; perform telephone triage; provide patient education.
• Escalate urgent clinical concerns and document interactions in the EHR.
• Coordinate referrals, testing, patient outreach, and prescriptions to support continuity of care.
• Maintain complete clinical documentation and comply with HIPAA and OSHA standards.
