Smithfield Rehabilitation and Care Center in Smithfield, NC is seeking an experienced Treatment LPN to deliver expert-led wound care within a collaborative team. You will assess wounds, perform dressing changes, debridement when needed, and document progress in the EMR system.

The role requires a current NC LPN license and CPR/BLS certification, with opportunities for ongoing training and professional development in a supportive work environment.

Qualifications

• Current and unencumbered LPN license in North Carolina.
• Experience in wound care or wound-care certification preferred.
• CPR/BLS certification required.

Responsibilities

• Conduct comprehensive wound assessments and develop treatment plans.
• Administer wound care treatments including dressing changes and documentation.
• Monitor healing progression and communicate findings with the interdisciplinary team.
• Collaborate with providers, nursing staff, and therapists to deliver coordinated care.
• Educate staff, patients, and families on wound care management and prevention.
• Maintain accurate clinical records in the EMR system.
• Adhere to infection control and safety protocols.

Key Responsibilities

1. Conduct comprehensive wound assessments and develop individualized treatment plans
2. Administer wound care treatments including dressing changes, debridement, and documentation
3. Monitor wound healing progression and communicate findings with the interdisciplinary team
4. Collaborate with providers, nursing staff, and therapists to deliver coordinated care
5. Educate staff, patients, and families on wound care management and prevention
6. Maintain accurate, detailed, and timely clinical records in the electronic medical record system
7. Adhere to infection control and safety protocols in all aspects of wound care delivery

Qualifications

1. Current and unencumbered LPN license in the state of North Carolina
2. Previous experience in wound care or completion of wound care certification preferred
3. Strong clinical knowledge of wound healing, infection control, and pressure ulcer prevention
4. Excellent communication and collaboration skills
5. Familiarity with PointClickCare (PCC) or other EMR systems is a plus
6. CPR/BLS certification required
