Job summary

Canterbury Rehabilitation and Healthcare Center in Phenix City, AL is seeking an experienced Licensed Practical Nurse (LPN) to provide expert wound care and patient-focused support. You will conduct wound assessments, administer treatments, and collaborate with an interdisciplinary team to improve patient outcomes.

Ideal candidates hold a current LPN license for Alabama, with wound care experience or certification. CPR/BLS is required, and familiarity with EMR systems like PCC is a plus.

Qualifications

• Current and unencumbered LPN license in the state of Alabama.
• Wound care experience or wound care certification preferred.
• Strong knowledge of wound healing, infection control, and pressure ulcer prevention.
• Excellent communication and collaboration skills.
• Familiarity with PointClickCare (PCC) or other EMR systems is a plus.
• CPR/BLS certification required.

Responsibilities

• Conduct comprehensive wound assessments and develop individualized treatment plans.
• Administer wound care treatments including dressing changes, debridement, and documentation.
• Monitor wound healing progression and communicate findings with the interdisciplinary team.
• Collaborate with providers, nursing staff, and therapists to deliver coordinated care.
• Educate staff, patients, and families on wound care management and prevention.
• Maintain accurate, detailed, and timely clinical records in the EMR system.
• Adhere to infection control and safety protocols in wound care delivery.
