Job summary

Five Oaks Rehabilitation and Care Center in Concord, NC is seeking a Treatment Licensed Practical Nurse (LPN) to lead wound care initiatives and deliver expert, patient-centered care in a supportive team environment.

You will conduct assessments, implement individualized treatment plans, and collaborate with physicians, nurses, and therapists to improve outcomes while maintaining compliance and documentation in PCC. Excellent communication and wound care knowledge are essential.

Qualifications

• Current and unencumbered LPN license in North Carolina.
• Experience in wound care or certification preferred.
• Strong knowledge of wound healing, infection control and pressure ulcer prevention.
• Excellent communication and collaboration skills.
• Familiarity with PointClickCare or other EMR systems is a plus.
• CPR/BLS certification required.

Responsibilities

• Conduct comprehensive wound assessments and develop individualized treatment plans.
• Administer wound care treatments including dressing changes, debridement, and documentation.
• Monitor wound healing progression and report findings to the interdisciplinary team.
• Collaborate with providers, nursing staff and therapists to deliver coordinated care.
• Educate staff, patients and families on wound care management and prevention.
• Maintain accurate, detailed clinical records in the EMR system.
• Adhere to infection control and safety protocols in wound care delivery.

4. Excellent communication and collaboration skills
5. Familiarity with PointClickCare (PCC) or other EMR systems is a plus
6. CPR/BLS certification required
