Job summary

Public Health Management Corporation seeks a Case Manager for the Philadelphia Treatment Court program. You will coordinate services for a caseload of 50+ participants, oversee monthly supervision, and act as a liaison between treatment facilities and the court to share progress and ensure compliance.

Qualifications

• Bachelor's degree with 18+ credits in psychology, social work, sociology, criminal justice, or related field.
• Minimum of 1 year of direct service in counseling or case management in a substance use setting.
• Preferred: Experience with behavioral health conditions.
• Preferred: Experience with forensic backgrounds.
• Preferred: Experience presenting in a court setting.

Responsibilities

• Provide case management services to individuals with current forensic affiliations who abuse illicit psychoactive substances.
• Supervise each participant monthly or more often if needed to meet recovery needs.
• Serve as liaison between treatment facility and legal team; inform all parties of progress.
• Prepare summaries about participants’ progress and present at bar of court on selected Wednesdays.
• Ensure delivery of comprehensive services in housing, education, vocation, HIV, and recovery-oriented community services.
• Maintain written documentation of services in each participant’s record/chart per agency standards.
• Document interactions in CRS and PAJCIS databases, including calls and status updates.
• Prepare graduation packets for participants completing Phase IV.
• Provide monthly caseload statistics after graduations and for potential graduates.
