Job summary

PHMC’s Philadelphia Treatment Court Case Management team seeks a dedicated Case Manager to support 50+ participants through court appearances, counseling, and recovery services. You will coordinate housing, employment training, medical treatment, and ensure timely documentation in client records.

Qualifications

• Bachelor's degree required with 18 credits in psychology or related field.
• Minimum 1 year of direct service experience in counseling or case management in a substance use treatment setting.
• Preferred: Experience with behavioral health conditions.
• Preferred: Experience presenting in a court setting.

Responsibilities

• Provide case management services to individuals with forensic affiliations who abuse psychoactive substances.
• Supervise participants monthly or as needed at the Case Management Office to assist recovery needs.
• Liaise between treatment facilities and legal team to inform on progress and issues.
• Prepare summaries of participants' progress and present to court on selected Wednesdays.
• Ensure delivery of housing, education, vocation, HIV, and recovery services.
• Maintain documentation in participant records per agency standards and licensing requirements.
• Document interactions in CRS and PAJCIS databases, including status updates.
• Prepare graduation packets for participants completing Phase IV and assist with monthly statistics.
