Jewish Family and Children’s Service seeks a Treatment Coordinator to provide ongoing care coordination for clients with behavioral health needs. You will help families access community resources, address barriers, and coordinate with schools, DCS, and other entities to support discharge and aftercare planning.

Treatment Coordinator: The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing behavioral health needs. This position assists clients and families in obtaining community support, identifying resources, and resolving barriers.
