Job summary

JEWISH FAMILY AND CHILDREN’S SERVICE INC in Mesa, AZ is seeking a Treatment Coordinator to provide ongoing care coordination for clients with behavioral health needs, coordinating resources and supporting families.

The role involves crisis response, collaboration with schools and agencies, and ensuring smooth transitions through discharge and aftercare, emphasizing teamwork and strengths-based practice.

Qualifications

• Bachelor's Degree in a human services field preferred; a combination of experience and education may be considered.
• Relevant experience working in a behavioral-health setting.
• Verification of education, employment, references.
• MVR matching JFCS driving guidelines and AZ Driver's License with valid auto insurance.

Responsibilities

• Performs ongoing coordination of services to children and adults in accordance with the treatment plan.
• Responds to initial crisis calls and identifies needs & necessary resources accordingly.
• Engages formal and informal resources for the client, including transition, discharge, and aftercare plans.
• Provides coordination of behavioral-health services with agencies such as DCS, Probation/Parole, DDD, and schools.

Salary Range: $22.00 To $23.00 Hourly

Treatment Coordinator: The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing behavioral health needs. This position assists clients and families in obtaining community support, identifying resources, and resolving barriers.

• West Valley Behavioral Health Center

Responsibilities:

• Performs ongoing coordination of services to children and adults in accordance with the treatment plan.
• Responds to initial crisis calls and identifies needs & necessary resources accordingly.
• Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services while demonstrating a positive, strengths-based attitude of teamwork and shared leadership.
• Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers.
