Job summary

Upstate Caring Partners in New York seeks a Case Manager I for the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. You will provide person-centered case management and connect individuals with housing and behavioral health services.

You'll support participants in building life skills, accessing housing, and maintaining wellness, working with treatment teams and families while ensuring safety and regulatory compliance.

Qualifications

• Bachelor's degree in a human-services field, OR
• Associate's degree + 3 years of experience, OR
• High school diploma + 5 years of experience in human services.
• Minimum 2 years in case management or housing support for individuals with mental health/substance use needs preferred.
• Prior experience in OMH Housing Programs strongly preferred.
• Valid NYS Driver's License and reliable transportation.

Responsibilities

• Implement individualized treatment and rehabilitation plans.
• Provide consultation and foster peer support among participants.
• Ensure safety and upkeep of residences, equipment, and supplies.
• Assist individuals in securing and maintaining stable housing.
• Conduct assessments and develop service plans.
• Coordinate tenancy support, including conflict resolution and benefit navigation.
• Maintain accurate documentation and comply with agency and regulatory standards.
• Provide crisis support in various community settings.
• Work closely with treatment teams, families, and external partners.
• Build and maintain relationships with landlords and community providers.
• Support internal and external reporting requirements.

Education

Bachelor's degree in a human-services field

Associate's degree + 3 years of experience

High school diploma + 5 years of experience in human services
