Job summary

Upstate Caring Partners is seeking a Case Manager I for the Treatment Apartment Program (TAP), an OMH-licensed scattered-site housing initiative in Central New York. You will provide person-centered case management and connect individuals with housing, housing supports, and behavioral health services to promote recovery and independence.

You'll develop individualized service plans, coordinate tenancy supports, and work with treatment teams, families, and community partners to ensure safety,

Qualifications

  • Bachelor’s degree in a human-services field or equivalent experience.
  • Minimum 2 years of case management or housing support experience preferred.
  • Prior OMH Housing Programs experience is strongly preferred.

Responsibilities

  • Implement individualized treatment and rehabilitation plans.
  • Provide consultation and foster peer support among participants.
  • Ensure safety and upkeep of residences, equipment, and supplies.
  • Assist individuals in securing and maintaining stable housing.
  • Conduct assessments and develop service plans.
  • Coordinate tenancy support, including conflict resolution and benefit navigation.
  • Maintain accurate documentation and comply with agency and regulatory standards.
  • Provide crisis support in various community settings.
  • Work closely with treatment teams, families, and external partners.
  • Build and maintain relationships with landlords and community providers.
  • Support internal and external reporting requirements.

Are you passionate about supporting individuals on their journey to recovery and independence? Join our dedicated team as a Case Manager I in the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. This role is essential in providing person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability.

You’ll help participants build life skills, access housing and behavioral health services, and foster community and family connections—all while promoting wellness and stability.

Core Responsibilities
  • Implement individualized treatment and rehabilitation plans.
  • Provide consultation and foster peer support among participants.
  • Ensure safety and upkeep of residences, equipment, and supplies.
  • Assist individuals in securing and maintaining stable housing.
  • Conduct assessments and develop service plans.

  • Prior experience in OMH Housing Programs strongly preferred.
  • Valid NYS Driver’s License and reliable transportation.

This position typically follows a Monday–Friday, 8:00 AM to 4:30 PM schedule, with flexibility as needed to meet program and participant needs. Certain deadlines and unanticipated developments may require work during evenings, weekends.
