This role is essential in providing person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability.
You'll help participants build life skills, access housing and behavioral health services, and foster community and family connections - while promoting wellness and stability.
This position typically follows a Monday-Friday, 8:00 AM to 4:30 PM schedule, with flexibility as needed to meet program and participant needs. Certain deadlines and unanticipated developments may require work during evenings, weekends.
