Job summary

Elara Caring is hiring a Speech-Language Pathologist to deliver high-quality in-home care across our service area in Manchester, NH. You will assess and treat patients in their homes, coordinate with family and the care team, and help advance the patient’s communication goals within an established care plan.

The ideal candidate has a Master’s degree in Speech-Language Pathology, an active SLP license in New Hampshire, and at least one year of clinical experience.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of clinical SLP experience.
• Willingness to travel up to 50%.
• Ability to lift up to 50 pounds and assist with patient transfer.

Responsibilities

• Promotes Elara Caring’s philosophy, mission statement and policies to ensure quality of care.
• Evaluates the patient and develops a plan of care for speech-language pathology needs.
• Admits the patient and assesses therapy needs per OASIS/PPS guidelines.
• Conducts initial evaluations and follow-up assessments of Speech-Language pathology needs.
• Implements and revises the Plan of Care in coordination with physicians and care team.
• Instructs patients and caregivers in home therapy programs.
• Keeps accurate therapy notes and logs and ensures documentation is complete.
