Job summary

Elara Caring is hiring a Speech-Language Pathologist to join our home-health team. You will assess and treat speech-language disorders in patients in their homes, evaluating needs and creating personalized care plans.

Responsibilities include initial evaluations, ongoing therapy, and coordination with physicians and care teams to ensure quality and continuity of care. This role requires travel within the service area and adherence to agency policies.

Qualifications

• Master’s degree from an accredited program in Language Pathology.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of clinical SLP experience.

Responsibilities

• Promotes Elara Caring’s philosophy and policies to ensure quality of care.
• Evaluates patient needs and develops a Plan of Care accordingly.
• Admits patient and assesses needs per OASIS/PPS guidelines.
• Conducts initial and ongoing speech-language therapy evaluations.
• Leaves comprehensive notes and coordinates with physician and team.
