Elara Caring is seeking a Speech-Language Pathologist dedicated to delivering high-quality, patient-centered care in home settings. You will evaluate, plan, and deliver speech-language therapy across our patient base, coordinating with physicians and care teams to ensure seamless service.

The role requires a Master’s in Speech-Language Pathology, an active SLP license, and at least one year of clinical experience. Travel up to 50% and a valid vehicle are essential.”

Qualificações

• Master’s degree from an accredited program in Language Pathology.
• Current, unrestricted SLP license in the work state.
• Minimum one year of experience as an SLP in a clinical setting.
• Able and willing to travel within branch/office coverage area.

Responsabilidades

• Promotes the organization's mission and care quality.
• Evaluates patients to determine rehabilitation needs and creates a plan of care.
• Performs initial and ongoing speech-language pathology assessments.
• Initiates patient safety plans and coordinates with family and community resources.
• Communicates findings to supervisors and the care team.
• Documents plans of care and ensures discharge planning.
• Provides direct therapy adhering to standards and physician orders.
• Instructs patients/caregivers in home therapy and ensures continuity of care.
