Job summary

CVS Health Corporation is seeking an experienced Registered Nurse Case Manager to provide telephonic remote care for members with complex healthcare needs. You will assess, plan, coordinate, and manage care while helping members access resources and improve outcomes.

The role offers a 40-hour work week, Monday–Friday hours, and no nights or weekends required. Salary ranges from $54,095 to $142,576 with bonus opportunities and a comprehensive benefits package.

Qualifications

• Active, unrestricted RN license in state of residence.
• Willingness to obtain a Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Associate Degree in Nursing (ADN).
• Ability to work within Monday-Friday business hours.

Responsibilities

• Conduct telephonic assessments and develop individualized care plans.
• Identify health risks, barriers, and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to clinical programs and resources.
• Apply clinical judgment to support members with complex conditions.
• Collaborate with multidisciplinary teams to achieve outcomes.
• Maintain compliance with regulatory requirements and company policies.
