Job summary

Harmony Home Health and Hospice, LLC. is seeking a Part Time Physical Therapy Assistant in Albuquerque, NM. You will assist in implementing the rehabilitative plan of care under the direction of a licensed Physical Therapist and participate in ongoing patient care and documentation.

Responsibilities include initiating therapy after PT instruction, guiding home exercise programs, and coordinating with the PT on patient progress while adhering to safety and confidentiality standards.

Qualifications

• Graduate of a Physical Therapy Assistant program approved by the APTA.
• State licensure or certification in the practicing state.
• One year of healthcare experience preferred.
• Ability to read and follow written instructions and document care.
• Current CPR certification and ability to pass background check.

Responsibilities

• Contact patient within 24 hours and begin therapy as prescribed.
• Administer therapeutic exercises and techniques delegated by the PT.
• Instruct home exercise programs to patient and family.
• Assist patients in using assistive devices.
• Consult with the PT on patient progress and outcomes.
• Attend department staff meetings and case conferences.
• Maintain confidentiality and professional conduct per Harmony policies.
• Document patient care according to established criteria and schedule.
• Follow OSHA guidelines and maintain safe work practices.
