Job summary

Otterbein SeniorLife is seeking a Physical Therapist to provide rehabilitative care to home health patients in the Lebanon, OH area, traveling locally to Tipp City, Dayton, and Middletown. You will work with an interdisciplinary team to implement and adjust home-based therapy plans, focusing on ambulation, transfers, gait training and home exercise programs.

The role requires a Bachelor's or Master's in Physical Therapy, Ohio licensure or active application, valid driver's license, CPR

Qualifications

• Bachelor’s degree or master's in physical therapy.
• Current license as a Physical Therapist from the State of Ohio or application in process.
• Valid Ohio driver’s license and reliable transportation.
• Current CPR certification.
• Complete a minimum of twelve (12) hours of in‑services per year.

Responsibilities

• Provide physical therapy services to patients according to a plan of care, including evaluation and direct treatment.
• Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances.
• Report patient’s reaction to treatment or changes in care to the physician as needed.
• Initiate physical therapy programs and instruct others in their therapy program and goals.
• Supervise the Physical Therapist Assistant (PTA) as necessary.
• Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.
• Confer with attending physician and home care team to promote coordinated care.
• Prepare and submit clinical and progress notes in a timely manner and in accordance with guidelines.
