Otterbein SeniorLife is recruiting a Physical Therapist for our Universal Home Health program in the West Liberty, OH area. The role involves traveling to patient homes to deliver comprehensive physical therapy services, assess and advance mobility, and educate patients and families on home exercise programs.

The position supports an interdisciplinary home care team and requires a current Ohio PT license, valid driver’s license, and CPR certification.

Qualifications

• Education: Bachelor’s degree or Master’s in physical therapy.
• Experience: Minimum 2 years of physical therapist experience, home health preferred.
• Licensure: Current Ohio Physical Therapist license; valid Ohio driver’s license; CPR certification.
• In-services: At least 12 hours per year.

Responsibilities

• Provide physical therapy services per plan of care and participate in plan development.
• Assess muscle strength, mobility, gait, ROM, and transfer capabilities.
• Instruct patients and families on use and care of therapies and appliances.
• Report changes to physician and care team as needed.
• Initiate therapy programs and educate staff and families.
• Supervise PTA as applicable.
• Participate in quality assessment/improvement initiatives.
• Coordinate care with home health team to ensure comprehensive services.

Otterbein Home Health is a Medicare-certified home health agency with an established reputation for high-quality services. Otterbein Home Health services are an integral part of Otterbein Retirement Living Communities services portfolio and available to Otterbein residents, in addition to residents in the surrounding area.

This position travels locally throughout the greater West Liberty Oh area (Auglaize, Logan, Champaign, and Hardin counties) providing patient services in a Home Health care setting.

Providing a high level of personalized care, Home Health by Otterbein is guided by Otterbein's nearly century-old tradition of faith-based ministry, clinical skills and strong community ties.

The Physical Therapist provides rehabilitative care (physical therapy) for home health patients needing assistance and training in ambulation, transferring, gait training and home exercise programs. The Physical Therapist works as part of the interdisciplinary home care team including the Registered Nurse, Physical Therapist, Speech Therapist, Social Worker and Home Health Aide.

Shift: Part Time and PRN

Responsibilities

• Provide physical therapy services to patients according to a plan of care. This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment.
• Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances.
• Report patient's reaction to treatment or changes in care to patient's physician as needed and appropriate.
• Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy.
• Supervise the Physical Therapist Assistant (PTA) as necessary.
• Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.
• Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care.
• Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Universal Home Health, state and federal guidelines and regulations.

Education and Experience

• Education: Bachelor's degree or master's in physical therapy.
• Experience: Minimum 2 years of physical therapist experience with community / home health preferred.
• Licensure, Registration, and/or Certification: Current license as a Physical Therapist from the State of Ohio or made application for same. Must have a valid Ohio driver's license and access to reliable transportation. Current CPR certification required.
• Complete a minimum of twelve (12) hours of in-services per year.
