Job summary

Otterbein SeniorLife in Lebanon, Ohio seeks a Certified Occupational Therapist to provide skilled home health services in patients' residences. Qualifications

• Graduate of an OT curriculum accredited by the AMA/AAOTA
• Current licensure as an OT in Ohio
• Minimum two years’ OT experience (home health preferred)
• CPR certification and driving eligibility for home visits

Responsibilities

• Performs initial evaluation and ongoing patient assessments per physician orders
• Initiates and updates plan of care; administers occupational therapy modalities
• Documents care in compliance with laws and agency policies
• Participates in case conferences with multidisciplinary team
• Supervises COTAs and ensures patient care needs are met
• Coordinates DME/safety devices with patient and family
• Facilitates discharge planning and community resource referrals
• Maintains confidentiality and communicates status changes to the care team
